As far as the visitation goes I am not sure if you are not letting dad see the child or if he is just writing not showing up so i can not advise further other than you do not want to writing be in contempt of court on a visitation order. You really need specifics in your court order writing (thus the parenting plan I explained) Best way to do that is to have one drawn up and send it back and forth until you guys can agree on things. If you are close but not quite on target with some things perhaps a mutual respected friend or relative can help mediate the rest of things so you wont have to spend money getting things resolved. The only people that come out on top in situations like these are the attorneys that make money on other peoples unhappiness. try to work things out like adults.
